1. School Physical (dated after September 1, 2021) on the State of Illinois Child Exam Form
2. School Dental Exam
3. Immunizations (SEE: IMMUNIZATION REQUIREMENT PAGE) must be up to date with proper documentation BEFORE the first day of school!

All incoming 12th grade students must provide receipt of receiving the Meningitis (MCV4) vaccine BEFORE beginning your senior year!
