ATTENTION INCOMING 2029 FRESHMAN STUDENTS:
1. All incoming freshmen are required to have an updated
Certificate of Child Health Exam (dated after September 1, 2024) on the State of Illinois Child Exam Form (no IHSA sports physicals accepted as school physicals)
2. School Dental Exam
3. Immunizations (SEE: IMMUNIZATION REQUIREMENT PAGE) must be up to date with proper documentation BEFORE the first day of school!

ATTENTION JUNIOR STUDENTS:
All incoming 12th grade students must provide receipt of receiving the Meningitis (MCV4) vaccine BEFORE beginning your senior year!
